Set-up fee: $50.00 ($100.00 non-members)
Per claim fee: $2.75 ($2.95 non-members)*
$15.00 flat rate for submission of less than 5 claims per month (non-members only)

Quantity Discount Program:

50-74** claims per month earns a 5% discount.
75-99** claims per month earns a 10% discount.
100** or more claims per month earns a 15% discount.

* For billing purposes, services and materials are considered separate claims.
**Note: Qualifying monthly quantities subject to change.
